The book version of the 2009 album 'Absolute Greatest' featured two pages which contained a total of 140 singles sleeves from around the world, and this page lists the origins of each.

For 10 sleeves, the single is known but the country is not, and for the single sleeve which follows (which appears as the second image on the bottom row of the first page), the country is known but the actual single itself is not. Apparently this single originates from Poland, as the 'Tonpress' record label logo can just about be seen in the bottom right hand corner.
